What Is All-on-4?

All-on-4 is a full-arch tooth replacement technique that uses four strategically positioned  dental implants to support a complete, fixed set of upper or lower teeth. Instead of replacing each missing tooth individually, the implants support a full-arch prosthesis.

All-on-4 may be suitable for patients who have lost most or all of their teeth in an arch, or who currently wear removable dentures and are looking for a more stable, fixed alternative.

The implants are positioned and angled to make the best use of the available bone in the jaw. In some cases, this approach may reduce the need for extensive bone grafting, particularly for patients who have experienced bone loss. However, suitability depends on each patient’s individual bone structure and clinical condition.

All-on-4 is one approach to supporting a full arch of replacement teeth using dental implants. Suitability depends on clinical examination, bone condition, oral health and other individual factors. In selected patients, a full-arch prosthesis may be supported by several dental implants. The number and position of implants depend on bone condition, anatomy, bite forces and the proposed prosthesis. Typically, full arch reconstruction of edentulous ridges requires five to 10 dental implants; however, some patients considering a fixed implant-supported prosthesis and in selected cases, tilted or strategically positioned implants may be considered where grafting is unsuitable or not preferred.

This approach is not appropriate for every patient. These situations could pivot the treatment plan toward the All-on-4 protocol.

Provisional teeth may sometimes be placed soon after implant surgery when clinical stability and other conditions permit. Immediate loading is not suitable in every case.

Treatment aims may include support for chewing, speech and appearance. Outcomes vary and adaptation is required.

Increased esthetic awareness

A provisional prosthesis may be fitted on the day of surgery or later in selected cases. This depends on implant stability, bone condition and the treatment plan.

Am I a Candidate for All-on-4?

All-on-4 is generally suitable for patients who:

  • Are missing most or all of their teeth in the upper or lower jaw
  • Currently wear a full or partial removable denture and are considering a fixed alternative
  • Have experienced bone loss and may benefit from an implant approach that makes use of the available bone
  • Are in suitable general health for implant treatment
  • Do not have untreated gum disease or other oral health conditions that could affect treatment

Suitability cannot be determined based on these factors alone. An individual assessment is required, including a clinical examination and appropriate dental imaging, such as a CT scan, to evaluate your bone structure, oral health and overall suitability for treatment.

Treatment Timeline

One of the potential advantages of All-on-4 is that, in suitable cases, a temporary fixed prosthesis may be fitted on the same day. A typical treatment journey may include: 

  1. Consultation & planning — Your dentist will assess your oral health, conduct the necessary examinations and imaging, and develop a personalised treatment plan.
  2. Surgery day — The four implants are placed, and where clinically appropriate, a temporary fixed prosthesis may be fitted on the same day.
  3. Healing period — The implants gradually integrate with the jawbone through a process known as osseointegration. This typically takes several months.
  4. Final prosthesis — Once healing and implant integration are complete, the temporary teeth are replaced with a custom-made final fixed prosthesis.
